As a courtesy to our patients, we require:
- On-Time Arrivals: Please arrive to your appointment on time. We schedule appointment times to be able to address your needs at the time of the visit. If you are late to your appointment, you will be rescheduled. This will be considered a missed appointment.
- New Patient Arrivals: In order to allow for the time it takes to complete new patient forms, we require that you arrive 15 minutes early to your appointment. If you arrive less than 15 minutes early and have not already completed your forms on the patient portal, you will be rescheduled. This will be considered a missed appointment. If you complete your new patient forms online we ask that you still arrive 5-15 minutes early to ensure that we have all paperwork completed.
- Timely Cancellations: If you need to cancel or reschedule your appointment, please give 24 hours’ notice so that we may fill your appointment slot.
- Compliance: Patients are allowed ONE missed appointment in a 12-month period. After the second missed appointment, you will be charged a fee of $30.00 that must be paid prior to scheduling another appointment.
